About the Role
Community-based physician rounding in subacute skilled nursing facilities
Flexible scheduling — you control your day
Low census: 10–20 participants seen once weekly
Lead clinical oversight and quality for PACE provider services
Collaborate with an experienced, supportive interdisciplinary team
Serve as a physician leader and mentor to NPs and other providers
Champion person-centered care for frail elderly participants

Qualifications
MD or DO; licensed to practice in Colorado
ABMS Board Certified — Internal Medicine or Family Medicine (geriatrics preferred)
DEA registration; ability to maintain hospital privileges
Experience in primary care with frail or elderly populations
Passion for mission-driven, whole-person care
